  To Whom Do I Report A Student’s Death?
 

Reports of a student’s death at any University of South Florida campus should be made with the Division of Student Affairs at the address shown below. This office is responsible for notifying members of the USF community and insuring timely response and action by appropriate university officials.

When reporting a student’s death, please provide the office with as much of the information below as possible:

· Student name

· Student identification number/social security number

· Name and address of parents, spouse, or next of kin; indicate any special circumstance (e.g.  

 parents at separate addresses, whether the student resided on- or off-campus, etc.)

· How you learned of student's death

· Date and cause of death

·Death certificate or obituary
