If you have forgotten your PIN, you can have us temporarily reset it to your date of birth (mmddyy). Fax us a signed OASIS PIN Reset Form to (813) 974-5271. We cannot reset your PIN at your verbal request over the telephone. You may also have your PIN reset in-person by bringing your photo ID to the Tampa campus Registrar's Office, SVC1034. Because your PIN at USF is your electronic signature, it cannot remain your date of birth. OASIS will prompt you upon initial login using your temporary reset to assign yourself a new PIN. Access to FACTS.org requires you first self-assign a new OASIS PIN.
