If you have officially been granted a legal name change, you must submit a Name Change Form to have your student academic records reflected under your new name. (Click here to download now.) Your request must include your Student ID (usually your U.S. Social Security number), your signature, and legal documentation for justification. Acceptable legal documentation includes your marriage license (no church issued documents), divorce decrees, a court-ordered legal name change, or a Florida driver's license under your new name.
