If your USF student academic records are reflected under an incorrect U.S. Social Security number either due to a data-entry error or as a result of a new or changed number issued you by the U.S. Social Security Administration, submit a signed Social Security Number Correction Form (click here to download now). Your signature and a copy of your official, signed Social Security card must accompany your form.
