Data Warehouse Access Request

Use this form to request access to the USF Data Warehouse. Because the Data Warehouse contains data from all of the primary administrative systems your application may be reviewed by the University Data Security Council (UDSC).

Complete this form and indicate your understanding and acceptance of the responsibilities stated below by clicking on the Submit button. An email will be sent to your Accountable Officer requesting their confirmation and approval of your request. Your Accountable Officer then replies to the email indicating their approval. The Accountable Office you indicate on this form must have signature authority for your unit. This will be verified against FLAIR/SAMAS records for accuracy.

The USF Data Administrator monitors these requests and when appropriate submits them to the UDSC for approval. When all approvals are received your request is forwarded to USFIT for account creation. You will receive notification as to the disposition of your request within five business days.

Access to the Data Warehouse is granted in three Data Access Levels (DAL).

  • DAL1 - Public: open to USF employees and affiliates as approved.
  • DAL2 - Restricted: open to users who require this access in order to perform their jobs.
  • DAL3 - Confidential: open to users with a legitimate educational interest in accessing student academic history.

A simple matrix lists the most common classes of data requested and their corresponding DAL. The Data Warehouse "Tech Documentation" is available if you require more detailed information to make your selection.

All access requests must be approved by the requestor's Accountable Officer. Requests for DAL2 and DAL3 access are reviewed by the UDSC. If access to the level you are requesting is not approved access may be granted at a more appropriate level based on the explanation you provide below.

Responsibilities Associated With Access to University Data:

  1. Data to which you have access in the conduct of legitimate University business may not be used for any other purposes, nor may you pass it on to any other person, in any form, for any purpose other than legitimate University business.
  2. You are responsible for the security of the data to which you have access. This includes your adherence to University, College, Campus, and departmental policies regarding access to data, keeping data, storage media, and printouts in secure locations, periodically changing passwords, and ensuring your workstation is not accessible to others who do not have legitimate access to it when you are not physically present.
  3. You are responsible for distinguishing between public, directory, and confidential information. For guidance, see the USF catalog and USF Policy and Procedure 0-106. Confidential information regarding students or staff may not be released in any personally identifiable format. Any public information may be displayed in either individual or aggregated format.
  4. The privacy and disclosure of student records is governed by State and Federal laws. Only the Office of the Registrar is permitted to release student information to a third a party. Direct all external requests for student data to the Office of the Registrar. Inappropriate release of student information to a third party is grounds for termination of access to administrative systems and disciplinary action including termination of employment.
  5. For consistency, official counts of students, employees, and other items are developed or maintained by the Data Administration & Reporting Team in the Division of Budget & Policy Analysis. Many of these are printed in the University Fact Book. It is your responsibility to use these official counts on surveys, news releases, grant proposals, and other documents.
